How do I prepare for my first treatment?

You will receive an General Intake Form via email to complete on your phone, computer, or tablet.  This must be completed prior to your appointment to allow the practitioner to review your medical history, injuries, areas of concern, or medications that may interfere or require special attention during your treatment.
Map the location and arrive 10 minutes early to ensure an on time start.
Make changes to your appointment at least 24 hours in advance of your appointment start time.
